Checklist

• Download and fill in the application form

• Two passport photos (50mm wide x 70mm high)

• Photocopy of ONE Identity card (health insurance card, drive license, immigration card or citizen card is acceptable)

• Photocopy of Social Insurance Number card

• Photocopy of certificate or diploma (with notarized English or French translation for overseas diploma)

• Payment in money order or endorsed cheque (please pay to CATA)
